By submitting this form, you agree that Boomers Insurance may contact you to provide information about the Program of All-Inclusive Care for the Elderly (PACE), explain how the program works, and assist you with the steps involved in connecting to a local PACE organization.

Boomers Insurance does not make eligibility determinations and does not approve or deny enrollment into PACE. All eligibility and enrollment decisions are made solely by the local PACE organization and the state administering agency.

Our role is to educate, guide, and assist, including helping you gather required information and coordinating your referral to the appropriate PACE provider in your area.
